CDC PLACES: Local Data for Better Health

PLACES was funded by the Robert Wood Johnson Foundation in conjunction with the CDC Foundation.

Releases used: 2022 (data updated 15 Jun 2023); 2023 (data updated 10 Jul 2024); 2024 (data updated 11 Sep 2025); 2025 (data updated 4 Dec 2025). For each release the "GIS Friendly Format" county, place and ZCTA files supply the estimates and intervals; the long-format county file supplies CDC's national row and measure text. Census tract estimates exist in PLACES but are not republished here.

US Census Bureau

Cartographic boundary files (2024 vintage, 1:20,000,000) for county and state outlines; 2020 Census ZCTA-to-county and ZCTA-to-place relationship files for the land-area shares that assign each ZIP Code to a county and name the places it overlaps. Public domain.

American Community Survey 5-year context (population, median age, income, insurance, education, poverty) is shown only when the site is built with a Census API key; it is currently not included. When shown, the notice "This product uses the Census Bureau Data API but is not endorsed or certified by the Census Bureau" appears with it.

Not used

No HUD ZIP-to-ZCTA crosswalk is used (it needs a HUD User token); ZIP Codes on this site are Census ZCTAs, which usually but not always match postal ZIP Codes. County Health Rankings and other secondary compilations are not used.
